The guy sent me a message on /r/GlobalOffensiveTrade saying he wanted to buy my Butterfly Knife for 150 USD via PayPal. I sent him the knife but then he blocked me and never sent the knife.​

How should I prove that it's him?

Because the URLs match & his name is in name history on the steamrep tool. It's also the profile linked on his rep thread, which he has now also edited.

Please provide uncropped screenshots of your inventory history (inventory->more->view inventory history) as well as your PayPal activity page.

He keeps changing his name.

Always upload your screenshots to the site using the "Upload a File" button.

And it doesn't matter if he changes his name. His ID64 will always link to his profile.
