Need help removing a UHC tag.

I was banned from UHC a few years back due to an admin abusing his powers, I simply changed my name to a famous player's (at the time) as a joke with my friends and he outright banned me without any form of warning. I looked into it and apparently if there's no proof that I did anything wrong that in some cases steamrep would remove the tag, would that be possible in my case? How should I go about doing this?

That link doesn't have UHC or MCT as an option under appeal type, should I go on and write a normal appeal despite it saying you guys don't handle UHC, and MCT is gone?
