Added user and requested trade, he added the item, Scree'auk's Talon, and I added the keys. However, he requested if he could have one rare as a 'sweet'. I added the Chronite Bracers without really thinking about it and hit accept twice without noticing that he had removed his item.

If you check his inventory you will see he has the items I gave him as well as still possessing the talon.

Just an update, looks like he has already traded away the Talon and the Keys for a Immortal Kunkka blade, which he has posted up for trade on dota2lounge.com.

Unfortunately, I didn't think to take a screenshot during the trade. I didn't even realize what had happened until afterwards. I have a screenshot of my trade history however, if that helps at all.

Also, did a quick search on the forums and looks like someone else has also been scammed by this user:
